"All on 4" replaces a full arch of teeth with a fixed bridge carried on four implants. Two of those implants are placed upright toward the front of the jaw and two are placed at the back at an angle, so that they sit in the denser bone nearer the front and stay clear of the sinus above and the nerve below.
The angled placement is what allows the technique to work where bone has already receded. Because the implants use the bone that is there, many patients avoid grafting, along with the months of healing a graft adds to the treatment.
Dr. Mathai provides both All on 4 and All on 6 dental implant prostheses at Lands End Dental and recommends one or the other after an assessment. If the implants are stable enough on the day of surgery, a fixed temporary bridge is fitted at the same visit. This is why this procedure is often referred to as "teeth in a day."
Patients with many teeth that cannot be saved, or with a mouth where saving them one at a time has stopped making sense either clinically or financially.
Patients wearing a denture that moves or rubs, or that has left them avoiding foods they used to eat.
Patients who have been told elsewhere that a bone graft is needed before implants are possible, since the angled placement can sometimes make that unnecessary.
All on 4 or All on 6? All on 4 is often the choice where there is bone loss toward the back of the jaw and a graft can be avoided. All on 6 tends to suit patients with good bone volume across the whole arch or a heavy bite from grinding or clenching. Which applies is decided at the consultation from the bone, the bite, the arch being treated and how the bridge will be designed.
The cost depends on whether teeth need removing beforehand, which material the final bridge is made from, and whether one arch is being treated or both.
You will receive a written plan with the cost of each stage at your consultation, before anything begins. Patients without PPO insurance can join the practice's Dental Discount Plan.
It depends on your mouth. The main factors are whether teeth need removing first, which material the final bridge is made from, and whether one arch is being treated or both. You receive a written plan with the cost of each stage at your consultation, before anything begins.
Neither is better as a rule. All on 4 uses angled implants at the back to work with less bone and often avoids grafting, while All on 6 spreads the load across two more implants and suits patients with good bone volume or a heavy bite. Dr. Mathai will recommend one or the other after assessing your jaw, your bite and how the bridge will need to be built.
Usually, in the form of a fixed temporary bridge that is screwed into place before you leave, provided the implants are stable enough on the day to carry it. The permanent bridge is made once the implants have integrated with the bone, which takes a few months.
Sometimes. The back implants are angled so that they sit in the denser bone toward the front of the jaw, which is the situation All on 4 was designed for. An assessment will show whether your bone allows it.
With reasonable hygiene and regular check-ups, implants can last many years. The bridge they carry may need maintenance, repair or replacement over time, and the review appointments are where that is picked up early.
